What lemborexant is
Lemborexant is the active substance of Dayvigo, a medicine registered in Japan for the treatment of insomnia. Unlike familiar sleeping pills that increase inhibition in the brain, it works on the system that keeps a person awake.
It is supplied as 10 mg tablets, 100 to a pack in ten blisters of ten. The original Japanese product is manufactured by Eisai Co., Ltd., a company based in Tokyo.
Lemborexant belongs to the dual orexin receptor antagonists. The class is fairly young: the first medicines of this type appeared only in the 2010s.
How lemborexant works
The orexin system
A small group of neurons in the brain releases orexin, a substance that sustains wakefulness. A lack of it causes narcolepsy, while excessive activity makes falling asleep difficult.
Receptor blockade
Lemborexant occupies both types of orexin receptor and prevents the wakefulness signal from getting through. As a result a person falls asleep faster and wakes less often during the night.
How it differs
Classic hypnotics act through the inhibitory GABA system and alter the architecture of sleep. Lemborexant acts differently, which is why it is placed in a separate group.
Approved uses
The label names the treatment of insomnia in adults, both difficulty falling asleep and difficulty staying asleep. Efficacy was assessed in studies that included a trial lasting more than six months.
Use in older adults has been studied separately, since insomnia is common in that group and sensitivity to hypnotics is higher. Caution with the dose, which a physician adjusts, matters especially here.
The medicine is not used in narcolepsy. Prescribing it for other sleep disorders, or alongside other conditions, is discussed with a specialist individually.
What to keep in mind
The most frequent unwanted effect is daytime sleepiness. Driving and operating machinery are acceptable only when you are fully alert, otherwise the risk of error, and with it the risk of injury, grows.
Rarely, medicines of this class are linked to complex sleep behaviours: a person may get up without being fully awake and have no memory of it in the morning. Such an episode should be reported to a doctor.
Alcohol and other medicines that cause drowsiness add to the effect of the drug. They are not combined with it without a doctor's approval.
