What linezolid is

Linezolid was the first member of the oxazolidinone class to reach clinical practice. The class emerged as a response to bacteria that had become resistant to several antibiotic families at once.

The molecule is fully synthetic: it does not copy a natural antibiotic but has its own structure and its own target inside the bacterial cell. That is why cross-resistance with penicillins, macrolides or fluoroquinolones is usually not seen.

The medicine is taken by mouth. It is absorbed well in the gut, and this allows tablets to be used where intravenous infusions were once required.

How linezolid works

The target

Linezolid binds to the 23S subunit of the bacterial ribosome, the part of the cell protein machinery where the peptide chain is assembled.

What follows

Once attached, it prevents the ribosome from taking the first step of synthesis. Protein is not built, and the bacterium loses the ability to grow and divide.

A note on the mechanism

Unlike many antibiotics, linezolid acts mainly at the initiation stage of translation rather than during its continuation. The effect is described as bacteriostatic: the organism stops multiplying, and the patient's own immune system completes the process.

Approved uses

The manufacturer's information lists bacterial infections of the skin and soft tissues, as well as pneumonia, including cases caused by organisms resistant to other antibiotics.

Infections where the organism is resistant to several agents, such as methicillin-resistant staphylococcus, are mentioned separately. Giving linezolid for a viral infection, including colds and influenza, makes no sense because it does not act on viruses.

Why linezolid is treated as a reserve drug

Linezolid is not a first-line choice for ordinary infections. It is held back for situations where the organism is resistant to simpler options or the patient cannot tolerate them.

That approach lowers the chance that bacteria quickly develop resistance to this molecule as well. Uncontrolled use of antibiotics speeds the process up, so the decision belongs to a physician.

During treatment, blood counts, liver function and neurological status are watched, because changes in these systems have been described with longer courses. The necessary checks are ordered by the doctor.
