What modafinil is
Modafinil belongs to the group of medicines that promote wakefulness. It is used in conditions where a person experiences pathological daytime sleepiness that interferes with study, work or ordinary life.
Unlike classic stimulants, modafinil has only a weak effect on dopamine release in the brain regions linked to pleasure. Its profile is therefore different: it sustains wakefulness rather than producing euphoria.
The substance dissolves poorly in water, so in the tablet it is present in a specially processed form that makes absorption more predictable.
How it works
The main target
Modafinil reduces the reuptake of dopamine and increases the activity of the systems that maintain wakefulness in the brain.
What happens
The signals that keep the cortex active persist longer. A person stays awake for longer and finds it easier to hold attention.
What it does not do
Modafinil does not replace sleep and does not remove the cause of the condition behind the sleepiness. It helps manage a symptom while the doctor treats the underlying problem.
Approved uses
The prescribing information lists three directions. The first is narcolepsy, in which attacks of irresistible sleepiness occur during the day.
The second is obstructive sleep apnoea, where the medicine is used as an addition to the main therapy rather than instead of it. The third is shift work sleep disorder, in which the sleep and wake cycle is disrupted.
The decision to prescribe, the length of use and the monitoring belong to a physician. The medicine is not meant to replace night rest in ordinary tiredness or sleep debt.
Monitoring during treatment
Modafinil affects the activity of liver enzymes, so it can change the effect of many other medicines. A doctor takes this into account when choosing companion treatment.
Heart disease and high blood pressure, as well as psychiatric conditions, need particular attention, and in these situations the decision is made especially carefully.
