What eltrombopag is
Revolade is a medicine whose active substance is eltrombopag. It is a small synthetic molecule rather than a protein, and it is taken by mouth. By its mechanism it belongs to the class of thrombopoietin receptor agonists.
Platelets are the cells that stop bleeding. When their number falls below a safe level the condition is called thrombocytopenia, and even a minor injury to a vessel becomes significant. Eltrombopag helps the bone marrow produce more of these cells.
The medicine comes as 25 mg tablets. A pack holds 14 tablets.
How eltrombopag works
The natural hormone
Platelet production is regulated by thrombopoietin. This hormone is made mainly in the liver and binds to a receptor on the surface of megakaryocytes, the large bone-marrow cells that later break up into thousands of platelets.
An unusual binding site
Eltrombopag attaches to a different part of the same receptor, not the site used by the natural hormone. As a result it does not displace thrombopoietin, and the two signals add up.
The signal inside the cell
Receptor activation switches on the JAK/STAT and MAPK cascades. Progenitor cells divide and mature more actively, and the number of fully functional platelets rises.
Approved uses
The manufacturer's information lists chronic immune thrombocytopenia in adults, severe aplastic anaemia and thrombocytopenia associated with chronic hepatitis C. In some situations the medicine is considered after earlier options have failed.
It is a prescription medicine. A haematologist decides on it from examination results, not a reference page. Using it on one's own is not acceptable.
Monitoring during treatment
Eltrombopag affects blood formation, so treatment is accompanied by regular tests. The doctor assesses platelet counts and other blood parameters and builds the approach on them.
